A Seminole woman was killed Monday in a two-vehicle head-on collision along SH 3, just north of Seminole.

The Oklahoma Highway Patrol worked the accident scene about 11:09 a.m. Trooper Brad Stuteville’s report shows Phyllis Tucker, 31, died at the scene of that accident.

The accident occurred as Tucker was driving a 2003 Pontiac Vibe northbound on SH 3. Troopers said a southbound 2010 Mercury Milan driven by Michael Rose, 34, Seminole, went left of center and struck Tucker’s vehicle head-on.

Troopers said the Mercury rolled three times and struck a mailbox, coming to rest on its tires, while the Pontiac rolled once and came to rest on its tires, the OHP said. Both drivers were pinned in the wreckage and had to be extricated by Seminole firefighters, the patrol reported.

Tucker was pronounced dead at the scene by Seminole firefighters and taken to Swearingen Funeral Home, the patrol reported. Her passenger, Jeremy Tucker, 31, Seminole, suffered a leg injury and was treated at Seminole Medical Center, then released.

Passenger Mason Tucker, one-month-old, was taken by helicopter ambulance to OU Children’s Hospital at OU Medical Center, where he was admitted for observation of possible head injuries, the report reads.

Rose also was taken by helicopter ambulance to OU Medical Center in Oklahoma City, where he was admitted in serious condition with multiple injuries.

Cause of the collision is listed as left of center in meeting, the report shows. As far as driver conditions at the time of the accident, Tucker’s is listed as normal while Rose’s condition is listed as being under investigation by the patrol.

Seat belts were in use in both vehicles.

Several troopers worked the scene with assistance from the Seminole Police Department, Seminole County Sheriff’s Department and Seminole Fire Department.
